The consultation starts with the questionnaire already read.
A booking in ClinicJourney is the start of a patient record, not just a slot in a calendar. The questionnaire goes out with the confirmation, and the answers that matter reach the practitioner before the patient sits down.
Where it goes wrong today.
Forms come back on the morning of the visit, or at the front desk.
Someone types a clipboard into the system while the patient waits, or the practitioner reads the history for the first time in the room.
The questionnaire is a PDF in an email.
It is not connected to the booking, so nobody can tell at a glance which patients tomorrow have not returned theirs.
Important answers are buried.
A previous treatment elsewhere, or a medication the practitioner wants to ask about, is on page three of four.
Booking, questionnaire and preparation are one sequence.
When a patient books, the record is created and the questionnaire your clinic wrote is sent with the confirmation. The front desk sees who has not returned it. Answers your clinic has chosen to mark for attention are listed for the practitioner.

- The booking and how it was made
- The questionnaire, as the patient answered it, with the version of your form
- Which answers your clinic marked for review
- The patient’s photo-use preferences
- Who prepared the consultation, and when
What the patient sees
A confirmation with the date, time and practitioner, and a link to the questionnaire. Sections they can finish in more than one sitting. A clear statement of what the clinic will do with their answers.
Where the line is
ClinicJourney shows the answers your clinic decided to mark, so a person reviews them. It does not interpret health information, assess eligibility, or decide whether a treatment is appropriate.
